AVID Prospective TEAM Form

This application is the first step in requesting to establish a new youth team within the AVID Soccer Club. The information in this form provides our Board of Directors with preliminary information to consider the feasibility of the request and determine whether the request fits within the mission of the AVID Soccer Club.

Upon submission of this application, the information will be reviewed by the Board of Directors. You will be contacted after this review with further information on how this request will proceed. Generally, after review by the Board, the applicant will be contacted and an in-person interview with the proposed team Head Coach and Team Manager will take place. Follow-up may take up to 7 days.
